Services includes: Eviction Prevention / Move-In Assistance Emergency Utility and Rental Assistance Senior Programs Information and Resources Nutritional Resources Transportation Information and Referral Housing Repair Referrals Other Emergency Services Provided as Resources Allow Case Management AppointmentsBeginning at 8 a.m. each Monday, call your nearest family service center to schedule an appointment. Family Services Centers are operated 7:30 a.m. to 5:00 p.m. Monday through Friday. EligibilityApplicants must:    Be a Phoenix resident    Provide proof of residency and citizenship Other program eligibility criteria vary Assistance may be restricted to once per 12-month period
